- You may schedule or reschedule your appointment via your Patient Portal 24 hours a day. Through the portal you may also request prescription refills, update personal information, review your medical records and communicate with staff on non-urgent health matters. If you have not activated your portal account or need assistance with access, please call 770.953.3331 and choose option #1. You will be directed to a staff member who will assist you.
- You may call our Centralized Scheduling Department at 770.953.3331 and choose option #1. Please note that at certain times of the day and seasons of the year, we experience a high call volume. You may be placed on hold and asked to leave a message for the scheduling department. Your call will be returned in the order it was received.
- Because we know your time is valuable, you may also request an appointment by filling out and submitting the form below. You will receive a return call from a staff member prior to close of business that day. If your appointment request is received late in the day or after hours, you will receive a call the following business day.
If your insurance has changed since your last visit, please have that information available when you speak with your scheduler.
