2017 Allergy Extract Order Deadline

Immunotherapy Patients: Please be advised that the last day to order allergy extract and have it billed to insurance in 2017 is Wednesday, December 20th. Any orders placed after this date will not be filled or billed until 2018. Guidelines for re-order prior to year-end:

1) PATIENT MUST BE ON MAINTENANCE DOSAGE

2) LAST REFILL MUST HAVE BEEN A MINIMUM OF TWO MONTHS PRIOR
