Thank you for your continued trust in our physicians and staff. We look forward to your return visit. To schedule an appointment, please choose one of the following options:

  1. You may schedule or reschedule your appointment via your Patient Portal 24 hours a day. Through the portal you may also request prescription refills, update personal information, review your medical records and communicate with staff on non-urgent health matters. If you have not activated your portal account or need assistance with access, please call 770.953.3331 and choose option #1. You will be directed to a staff member who will assist you.
  2. You may call our Centralized Scheduling Department at 770.953.3331 and choose option #1. Please note that at certain times of the day and seasons of the year, we experience a high call volume. You may be placed on hold and asked to leave a message for the scheduling department. Your call will be returned in the order it was received.
  3. Because we know your time is valuable, you may also request an appointment by filling out and submitting the form below. You will receive a return call from a staff member prior to close of business that day. If your appointment request is received late in the day or after hours, you will receive a call the following business day.

If your insurance has changed since your last visit, please have that information available when you speak with your scheduler.
